As a courtesy to our patients, we wish to provide you with our billing policy. Please note these highlights from our billing policies:
- Medicare - We are a Medicare participating provider. However, we are also required by Medicare regulations, to collect from the patient any co-insurance due and any unsatisfied deductible. As a courtesy, if you have a secondary insurance we will bill your insurance for the co-insurance and deductible.
- Medicare HMO's - many Medicare beneficiaries are changing their insurance from traditional Medicare to a Medicare HMO. If you have changed to a Medicare HMO, please remember to obtain a referral form from your primary care physician prior to your visit. If you do not have a referral when you arrive for your scheduled appointment, we will request that you reschedule your appointment. We apologize for any inconvenience. It is the responsibility of the patient to obtain a referral from the primary care physician. Please provide the front desk with your Medicare HMO card so that we may bill the proper insurance.
- HMO - If you have an HMO, please remember to obtain a referral form from your primary care physician prior to your visit. If you do not have a referral when you arrive for your scheduled appointment, we will request that you reschedule your appointment. We apologize for any inconvenience. It is the responsibility of the patient to obtain a referral from the primary care physician. All co-pays will be collected at the time of service.
- Payment is expected at time of service. We accept cash, check, Visa, Master Card, American Express, or Discover.
- Self-pay patients are requested to pay at the time of service. An initial payment of $150 must be made at the time of sign-in. This will be deducted from the amount of the actual visit.
- There may be a $25 charge for a missed appointment if not cancelled by 3:00 p.m. on the business day prior to the scheduled appointment .
- Patients with a copay are expected to pay at the time of sign-in. The visit will be rescheduled if the referral and copay are not presented on the day of the visit.
- Patients who require a referral must bring the referral with them for their visit. If the patient arrives without a referral, they will be given the opportunity to contact their primary care physician from the office. If they are unable to reach the physician to obtain the referral, they will be rescheduled for another day.

Surgical Procedures:
If you are scheduled for a surgical procedure, either in the office or in the hospital, we will bill your insurance company directly. Please remember that a few elective surgical procedures are not covered by insurance, including Medicare. If that is the case our staff will fully explain our payment policies for these procedures with you.
Inpatient Hospital Charges:
Charges for care rendered by DVU physicians in the hospital are billed through our office.
